Additive Implants Partner Portal

Additive Implants Partner Portal Terms of Use

Version 1.0, effective 2026-11-01. Interim version approved by Additive Implants pending counsel review; a revised version will be issued after review and you will be asked to accept it.

1. About these terms

These terms govern your use of the Additive Implants Partner Portal (the "Portal") operated by Additive Implants, Inc. ("Additive Implants," "we," "us"). By selecting "I accept" you agree to these terms on your own behalf and, where you use the Portal for an organization, on behalf of that organization. If you do not agree, do not use the Portal. These terms supplement, and do not replace, any distribution, sales representative, consignment, supply or other written agreement between your organization and Additive Implants (an "Agreement"). If these terms conflict with an Agreement, the Agreement controls.

2. Who may use the Portal

The Portal is for business partners of Additive Implants and their authorized personnel. Access is granted by Additive Implants after review and may be limited to certain areas based on your role and organization. You must be at least 18 years old and authorized by your organization to use the Portal.

3. Your account

4. No patient information

Do not enter, upload or send patient names, dates of birth, medical record numbers, images that identify a patient, or any other protected health information anywhere in the Portal, including forms, notes, messages and attachments. The Portal is not designed to store patient information. Report adverse events and product complaints through the channels named in the Portal; where a report needs patient details, Additive Implants will contact you to collect them by an approved route.

5. Confidential information

Information in the Portal, including pricing, statements, training, technical documents, plans and other business information, is confidential to Additive Implants and its partners. Use it only for your work with Additive Implants, share it only with people in your organization who need it for that work, and protect it at least as carefully as your own confidential information. These obligations continue after your access ends. Any confidentiality terms in an Agreement also apply.

6. Product information and documents

7. Training and certificates

Training records and certificates reflect completion of Additive Implants training only. They do not license or credential anyone to perform procedures, and they may expire or be withdrawn, for example when content is revised. Anyone may check a certificate's status using its code on the public lookup page.

8. Statements and business records

Account statements, order and request histories and similar records are provided for convenience. The records in Additive Implants' accounting system control if there is a difference. Tell us promptly if something looks wrong.

9. Requests, forms and messages

When you submit a form, request or message through the Portal, you confirm the information is accurate to the best of your knowledge and that you are authorized to submit it. Submitting a request does not mean it is approved. Product complaints and safety reports you submit are handled under Additive Implants' quality system and may be shared with regulators as the law requires.

10. Acceptable use

You must not:

11. Monitoring and support access

To keep the Portal secure and to support you, Additive Implants records sign-ins and actions in the Portal. Authorized Additive Implants staff may view your account read only, as you see it, to resolve a support request or check that access is correct; each such view is recorded with its reason.

12. Privacy

We use your personal information (such as name, business contact details and Portal activity) to operate the Portal and manage our business relationship. We do not sell it. You can ask for a copy of our privacy notice, or ask about your information, at portal-support@aimplants.com.

13. Notifications

The Portal sends notifications by email and in the Portal. Some notices, such as recalls, field safety notices, labeling revisions and required retraining, cannot be switched off while you have access. Notifications never contain patient information, bank details or dollar amounts; they tell you something happened and link to the Portal. Text message notifications are not offered at this time.

14. Intellectual property

The Portal and its content belong to Additive Implants or its licensors. We grant you a limited, revocable, non-transferable right to use them for your business with Additive Implants while you have access. Trademarks shown in the Portal belong to their owners.

15. Availability and changes

We aim to keep the Portal available but do not promise uninterrupted service. We may change, suspend or retire any part of the Portal. We may update these terms; when we do, we will ask you to accept the new version before you continue using the Portal.

16. Disclaimers and limitation of liability

To the extent the law allows, the Portal is provided "as is" and "as available," without warranties of any kind, whether express or implied. Approved product documents are governed by their own content and by your Agreement. To the extent the law allows, Additive Implants' liability arising from your use of the Portal is limited as set out in your Agreement, and where there is no Agreement, Additive Implants is not liable for indirect, incidental or consequential damages arising from use of the Portal. Nothing in these terms limits liability that cannot be limited by law.

17. Ending access

You may stop using the Portal at any time. We may suspend or end your access if you breach these terms, your role or Agreement ends, or for security reasons. Sections 4, 5, 9, 14, 16 and 18 continue after access ends.

18. Governing law

These terms are governed by the laws of the State of Arizona, without regard to conflict of laws rules. Where your Agreement sets out how disputes are resolved, that process applies.
